Your cosmetic options for improving the appearance of the neck depends on factors that include the the amount of fat, the amount of skin laxity, the existence of vertical platysma bands and separation, horizontal neck lines, and sun damage.

Prior to the introduction of Coolsulpting Mini, Kybella was our go-to treatment option for the removal of unwanted fat in the neck.  Kybella is injected into neck fat in a similar way that we inject Botox.  Kybella works by dissolving fat in the area of injection.  The treatment causes some mild to moderate inflammation that can partially tighten skin after the fat is reduced.  

Depending on the amount of fat, you may need 1 to 3 treatments.  There is minimal pain.  And there is no recovery.  Most patients will have mild to moderate swelling but may return to work immediately.  Results can be seen after 6-8 weeks

SkinTyte is an advanced nonsurgical skin tightening device by Sciton.  The device delivers broad band light which precisely heats and denatures skin collagen.  The collagen remolds and tightens the skin in the healing process.

SkinTye is purchased as a package of 4 or 6 treatments one month apart. There is minimal pain.  And there is no recovery.   Most patients will return to work immediately.  Results can be seen after 6 to 8 weeks.

Liposuction is a minimally invasive procedure that directly sucks out and removes fat.  Most areas of unwanted fat can be removed.  The most common areas, however, include the neck, abdomen, flanks, arms, and thighs.  Small areas such as in the neck or arms can often be treated with regional anesthesia in the office instead of general asleep anesthesia in the surgery center or hospital.  The most significant limitation is in patients with either a lot of fat or if there exists a lot of extra skin.  Surgical tightening is required in these situations.

Because liposuction is more of a surgical procedure, most patients will take 3-5 days off of work and be required to wear a surgical compression garment for about 4 weeks.

A neck lift is a surgical procedure provides more dramatic tightening of the neck skin.  A neck lift uses incisions hidden behind the base of the back of the ear starting near the earlobe.  It then extends upwards along the crease before turning to the back of the scalp.  

If liposuction is included, that is performed first.  The degree of skin excess is then measured before planning out the neck lift part of the procedure.  

The procedure is often done in the office under local anesthesia.  Patients who choose, can opt to have the procedure at the surgery center or operating room where they can have general anesthesia.

A neck compression wrap is required for one full week and part time for another week.  Most patients return to work in 10-14 days.

Depending on the degree of skin excess, some patients may “get away” with a limited incision neck lift as opposed to the traditional neck lift just described.  Either procedure can be performed under local anesthesia in the office.  The incision extends from near the earlobe up the the posterior ear crease and ends at the top of the ear but does not extend much if at all into the scalp hairline.  

The procedure is often done in the office under local anesthesia.  Patients who choose, can opt to have the procedure at the surgery center or operating room where they can have general anesthesia.

The platysma is a neck muscle that usually forms a sling under the chin.  Separation and sagging of the muscle can often be seen as a “turkey neck”.  This muscle separates in about 30% of people as they get older.  

If the separation and neck sagging is prominent, you may need a platysmaplasty as well as a neck lift.  The platysmaplasty requires a small incision under the chin.  The muscles separation is visualized and sutured back together.
