55yo male executive with large male breasts who came in for treatment options. He states that even as a college football player when he was young, he was embarrassed of his large breasts that he says have just gotten larger with age.

His exam shows that he has disproportionately large breasts compared to the rest of his body. Even though his is slightly overweight, the breasts are larger and extend to the lateral chest and back. Liposuction only procedure was performed on the breast as well as on the lateral breast, chest, and upper back.

Nothing was treated in the abdomen or flanks. The after result can be seen 2 weeks later with minimal bruising and swelling. Because there was not a lot of actual breast tissue ( only fat ) and because there was not a lot of breast droop, only liposuction was required. If this were not true, then extra incisions would have been required to cut out the breast tissue or extra skin. Final result with even better results should be seen over the next 3 months.

Patient experience

55 old male executive who complains of enlarged male breast. He states that he has always had large breasts that embarrassed him even when he was a football player in his youth. He states that his breast protrude in fitted clothes including his dress shirts or even his golf shirts. His exam shows that although he is slightly overweight, his breasts are disproportionately larger. He also has large amount of lateral breast fat extending to his back that also protrudes. His treatment was purely liposuction or liposculpture of the anterior breast as well as the sides and back. No work was done in the abdomen or flanks. The incisions are still visible as he only had the procedure 2 weeks prior

Breast enlargement on males

Gynecomastia is also referred to as male breast enlargement. It can be an embarrassing development regardless of your age. It is, however, much more psychosocially problematic in young adults and teenagers during the period of developmental self-awareness and self-confidence.

Large breast enlargement can be caused by fat, breast tissue, or both. The treatment for gynecomastia depends on the the proportion of fat and breast tissue and the overall size of their enlargement.

Approach

Large fat content gynecomastia – large fat content gynecomastia can be treated with simple liposuction. The breast fat can be removed and the remaining fat can be sculpted as needed.

Large breast tissue content gynecomastia – large amounts of breast tissue can only be removed by directly excision ( directly cutting it out ) as liposuction will not be able to penetrate breast tissue which is much harder than fat. In general, an incision is placed below the nipple areola complex and the breast tissue is visualized and removed.

Large amount of fat and breast tissue gynecomastia – If the large male breast is due to having both larger amounts of fat AND breast tissue, you will require both liposuction and direct excision

Procedure

Very large amounts of fat and breast tissue gynecomastia – If the large male breast is very large enought to stretch the skin and the weight is enough to cause the skin to cause breast sagging or breast droop, then a formal breast reduction procedure is required. This requires removal and tailoring of the skin in order to recontour the breast. The procedure is similar to a breast lift or reduction in women.
