The Mini Facelift is designed for patients with minimal to moderate sagging of the face and to some extent the neck.  The procedure works elevate the jowl, improve the jaw line, add more fullness to the cheek while reducing the appearance of the nasolabial fold and marionette lines.  

Treatment:

The procedure can be performed under local or general anesthesia.  Incisions are hidden in the hair or in the creases of the ear.  Excess skin is elevated and removed.  The deep tissue is also elevated and permanently sutured in a higher position.  Most patients will also benefit from adding volume using injectable fillers or fat transfer.  Most patients, if only having the mini facelift, will be back to work in about 7-10 days.

Knowing exactly what a facelift is can be extremely confusing as plastic surgeons will use the term loosely to include face and neck lifts and sometimes forehead lifts.  Others will “market” their techniques by giving their technique their own names such as the Beverly Hills Facelift or the South Beach Lift.  Most plastic surgeons define, however, a facelift as a mini facelift which takes care of the lower face and jawline ( see above ) combined with a neck lift ( which obviously takes care of the neck )

I think of a facelift as a mini facelift combined with a neck lift.  Your consultation will define exactly what you might need.  Some patients might need extra facial volume, while others might need the addition of a forehead lift or eyelid lifts.  See this live youtube video to see how a facelift ( face and neck lift ) plus a forehead lift are performed ( warning : live video )

The  face and neck lift is designed for patients with moderate to severe sagging of the face and necl.  The face portion of the procedure works to elevate the jowl, improve the jaw line, add more fullness to the cheek while reducing the appearance of the nasolabial fold and marionette lines.  The neck portion of the procedure elevates the loose and hanging skin of the neck.

The procedure can be performed under local or general anesthesia.  Incisions are hidden in the hair or in the creases of the ear.  Excess skin is elevated and removed.  The deep tissue is also elevated and permanently sutured in a higher position.  Elevating the * SMAS” or deep tissue provides strength and longevity to your results.  Most patients will also benefit from adding volume using injectable fillers or fat transfer.  Most patients, if only having the mini facelift, will be back to work in about 10-14 days.

The neck lift procedure is designed for patients with moderate to severe sagging of the neck skin.  For the most part, the incisions are placed behind the ear in the crease between the ear and the scalp, and extends into the hairline to make it relatively invisible.   Some patients will need removal of chin and neck fat by liposuction.  Some patients will need the joining of the midline muscle bands that have separated by platysmaplasty.  

The  face and neck lift is designed for patients with moderate to severe sagging of the neck.  Some patients with severe sagging will require a mini-facelift also in order to properly re-drape the neck skin; while other patients with only mild neck sagging may ONLY need a mini-facelift. 

Treatment:

The procedure can be performed under local or general anesthesia.  Incisions are hidden in the hair or in the creases of the ear.  Excess skin is elevated and removed.  The deep tissue ( platysma muscle ) is also elevated and permanently sutured in a higher position.  Elevating the * Platysma” or deep tissue provides strength and longevity to your results.  Most patients will be back to work in about 10-14 days.

The brows begin to fall as we age.  When the descent is more significant, the brows will push down on the upper eyelids and make them appear more closed off particularly in the lateral upper eyelids.  The falling brow also adds to the appearance of extra skin to the upper eyelid ( which improves when you simply raise the brow ).  The face compensates for lowering of the brows by activating the forehead muscles to raise the brows.  This physiological compensation results in hyperactive and visible horizontal creases of the forehead.  

 

Treatment:

The procedure can be performed under local or general anesthesia.  Incisions are hidden and camouflaged in the hairline in a zigzag incision. Excess skin is elevated and removed concentrating on the lateral brow.  Volume replacement in the temples can improve the results.  Most patients, if only having the forehead lift, will be back to work in about 10-14 days

The eyes are the windows to the soul.  Aging of the eyelids usually precedes aging in the rest of the face.  Aging of the upper eyelids usually includes developing extra skin that can hang on or over the eyelashes, and the protrusion of the eyelid fat pockets.  Forehead or brow droop makes this appearance worst.  Aging of the lower eyelids are due in large part to the loss of fat volume in the under-eye hollows and cheek which thins out, lengthens, and darkens the skin ( usually referred to as dark circles.  

Upper eyelid blepharoplasty removes the skin excess and fat pockets of the upper eyelid.  This cleans up and brightens the appearance and eyes.  

Lower eyelid blepharoplasty removes the skin excess and fat pockets of the lower eyelid.  Volume replacement, whether by injectable fillers or fat transfer is often added.  Skin lasers are also often beneficial to reduce the fine lines and texture while lightening the skin.  

Treatment:

The procedure can be performed under local or general anesthesia.  Upper eyelid incision is placed in the normal upper eyelid crease.  Lower eyelid incisions is placed just below the lash line ( subciliary incision ) or behind the eyelid ( conjunctival incision ).  Skin and fat pockets are removed.  There is almost always a lot of bruising.  Most patients will be back to work in about 10-14 days.

Volume restoration whether it is by injectable fillers such as Juvederm or Voluma, or by Fat Transfer, is an integral part of a natural facial rejuvenation procedure.  Fat transfer was the original liquid facelift.  Some people began to refer to fat as liquid gold.  Instead of spending lots of money on the expensive over-head that comes with using injectable fillers, abundant amounts of fat can be removed from your own body to be used to add volume to the face, hands, breasts, and buttock.  

Fat transfer is a common addition to my signature mini facelift and facelift procedures.  Fat can be added to the temples, under-eye hollows, cheek, chin, and lips.  

Benefits over facial fillers include the fact that the fat can last for several years, fat is usually abundant, and fat is less expensive because there is no commercial overhead.  Drawbacks include the fact that it is more surgical, can cause a significant amount of swelling and bruising, and requires time off of work.

Treatment:

The procedure can be performed under local or general anesthesia.  Incisions are hidden and small.   Fat can be obtained from the any part of the body but most often comes from the lower abdomen or inner thighs.  The fat is processed to remove other fluid and tissue components, placed in small injectable syringes, and re-injected in the targeted areas.  Most patients, if only having facial fat transfer, will be back to work in about 7-10 days.

The liquid facelift is one of my signature procedures.  My technique evolved from the overarching principle that facial aging begins with the loss of facial volume; therefore, the most natural ( and simple ) results in facial rejuvenation comes from replacing facial volume first before proceeding with a surgical lift.  

The liquid facelift uses facial fillers and/or fat transfer to add volume to the temples, cheeks, under-eye hollows, lips, chin, and jowl notch.  Each area might be treated with a different filler including Sculptra, Voluma, Juvederm.  Some patients might also benefit from Vollure or Volbella or Botox.

Once facial volume is restored ( natural and not over-done ) excess skin is lifted if necessary.  

 

Treatment:

The procedure takes about 15-30 minutes in the office.  There is negligible downtime.  Most patients can return to work the same day or the next day.  Common procedures performed at the same time include botox and facial skin lasers.

Neck liposuction is a simple procedure that can be performed in the office or operating room.  The procedure is obviously designed to reduce the fat content and appearance of the neck.  Fat removal is a common addition to a face or neck lift fat accumulates in the neck with age.  A neck lift might be necessary in some patients if after neck fat removal, the skin appears too loose to shrink on its own.  We can usually determine the need before surgery.  

Neck liposuction is more aggressive ( and guaranteed ) than fat removal by Kybella or Coolsculpting .   On the other hand, Kybella and Coolsculpting are non-surgical procedures that allow you to return to work immediately.

Treatment:

The procedure can be performed under local or general anesthesia.  A very small 2-3 mm incision is placed beneath the chin crease.  Excess fat is carefully suctioned out of the neck.  A compression wrap is required for 7 days.   Most patients, if only having the neck liposucition will be back to work in about 7-10 days.
